The Careful project involves research regarding interventions to improve medication safety. The general focus will be on the high risk elderly patient with polypharmacy both in cure and care institutions and in primary care. It is a joint program of the AMC, LUMC, UMCU, and UMCG. The following interventions will be studied using Adverse Drug Events (ADEs) as primary outcome parameter: 1) screening of medication profile by pharmacist and optimizing therapy together with general practitioner. 2) use of automated system for detection of potential ADEs in hospitalized patients. 3) application of a ward oriented pharmacy team to prevent prescribing errors and administration. The UMCG part of the study focuses on the feasibility of and conditions for successful implementation of interventions on prescribing errors in the elderly with polypharmacy. Three aspects are included: 1) the identification of potentially relevant determinants of successful implementation of interventions. 2) the relation between these determinants and the success of the implementation process (process evaluation) and outcome (ADEs averted). 3) an economic evaluation of the different approaches used in the interventions.
