The main objective is improving dementia care services for European citizens with a special focus on the development of best practice strategies for the transition from formal professional home care to institutional long-term nursing care facilities.

Hoofddoel van dit project is de zorg voor mensen met dementie in Europa te verbeteren, waarbij speciaal wordt gekeken naar het ontwikkelen van best practices voor de transitie van professionele thuiszorg naar residentiële zorg (in Nederland verzorgings- en verpleeghuizen).
